Patient Registration Instructions

Bon Secours Surgery Center At Harbour View offers patients the convenience and privacy of a secure, online registration process. If you are a new patient to our center, please go online today to complete your registration using the login information below. You will be asked about your health history, medications, and previous surgeries. If you are a returning patient, please update your online form. It’s important to complete or update your online registration as soon as possible so that your medical team will have time to review your information prior to your visit. We will call you if we have any questions or concerns. We look forward to seeing you soon!

New Patients:

1. Go online to: www.bshv-sc.com
2. Go to: Patient Registration at the top right of the page.
3. Go to: Create Your Account, Click on Register
4. Enter your demographic information to create your first Medical Passport
5. Answer the questions to complete your online health history.

Returning Patients:

1. Go Online to www.bshv-sc.com
2. Go to: Patient Registration
3. Go to Welcome Back – enter your original user name in the Welcome Back area and click next. Enter your password.
4. Once you have successfully logged back into your account, click on the button located on the top right of the page

Forgot Username or Password?

1. Click Sign In Help if you don’t remember the username you created.
2. Click Forgot Password if you do not remember your password
